Chlorpromazine(CPZ), marketed under the brand namesThorazineandLargactilamong others, is anantipsychoticmedication.It is primarily used to treatpsychotic disorderssuch asschizophrenia.Other uses include the treatment ofbipolar disorder, severe behavioral problems in children including those withattention deficit hyperactivity disorder,nauseaandvomiting, anxiety before surgery, andhiccupsthat do not improve following other measures.It can be givenorally(by mouth), byintramuscular injection(injection into a muscle), orintravenously(injection into a vein).
Chlorpromazine is in thetypical antipsychoticclass,and, chemically, is one of thephenothiazines. Itsmechanism of actionis not entirely clear but is believed to be related to its ability as adopamine antagonist.It hasantiserotonergicandantihistaminergicproperties.
Common side effects includemovement problems,sleepiness, dry mouth,low blood pressure upon standing, andincreased weight.Serious side effects may include the potentially permanent movement disordertardive dyskinesia,neuroleptic malignant syndrome, severe lowering of theseizure threshold, andlow white blood cell levels.In older people with psychosis as a result ofdementia, it may increase therisk of death.It is unclear if it is safe for use inpregnancy.
Chlorpromazine was developed in 1950 and was the first antipsychotic on the market.It is on theWorld Health Organization's List of Essential Medicines.Its introduction has been labeled as one of the great advances in thehistory of psychiatry.It is available as ageneric medication.
