Tacrolimus inhibits the first phase of T-lymphocyte activation. It binds to the immunophilin FKBP (FK-binding protein). The tacrolimus-FKBP complex binds to and inhibits calcineurin phosphatase, which suppresses the proliferative response of T cells — transcription and activation of lymphokine genes do not occur. In atopic dermatitis, tacrolimus is applied topically and inhibits local inflammation. It also acts by binding to steroid receptors, inhibits mast cell mediator release, and inhibits E-selectin expression in damaged blood vessels. Tacrolimus does not inhibit collagen synthesis, so it does not cause skin atrophy (unlike topical corticosteroids).
⚠️ Warnings
Patients on tacrolimus should have periodic monitoring of blood parameters, urinalysis, vision, cardiac function (ECG, blood pressure), liver and kidney function, fasting glucose, potassium, hematology, and coagulation — these are routine. Diarrhea significantly affects tacrolimus blood levels — patients must inform the physician immediately. Topical users should avoid intense UV exposure (sun, sunlamps) during treatment.
