Mifepristone, also known by its developmental code nameRU-486, is a drug typically used in combination withmisoprostolto bring about amedical abortionduring pregnancy.This combination is 97% effectiveduring the first 63 days (9 weeks) ofpregnancy, and is effective in the second trimester as well.It is also used on its own to treatCushing's syndromeor in low doses as anemergency contraceptive.
The most common adverse effects include abdominal pain, feeling tired, and vaginal bleeding.Serious side effects may include heavy vaginal bleeding, bacterial infection, and, if pregnant, birth defects.When used, appropriate follow-up care needs to be available.Mifepristone is primarily anantiprogestogen.It works by blocking the effects ofprogesterone, making both thecervixand uterine vessels dilate and causing uterine contraction.Mifepristone also works, to a lesser extent, as anantiglucocorticoidand diminishes the effects ofhypercortisolism.
Mifepristone was developed in France in 1980 and came into use there in 1987.It became available in the United States in 2000, for medication abortion, and in 2010, for Cushing's syndrome.It is on theWorld Health Organization's List of Essential Medicines.Mifepristone was approved in China in 1988, India in 2002, and Canada in 2017.
⚠️ Warnings
•Do not use this if your pregnancy is outside the womb (ectopic pregnancy). It will not cause an abortion in this case.
• It may cause dizziness. Do not drives, operate machinery, or do anything while taking this medication.
It should be used with caution in patients with kidney, heart, lung, liver, adrenal disease. Patients with diabetes, high blood pressure and porphyria should also inform their doctor about their illness prior to taking this medication.
