How grapefruit interacts with Warfarin
Warfarin is metabolized by several liver enzymes, principally CYP2C9 (for the more active S-warfarin enantiomer) and CYP3A4 (for R-warfarin). Grapefruit inhibits CYP3A4 but has limited effect on CYP2C9 — so the theoretical interaction with warfarin is weaker than with simvastatin or felodipine. Published studies are mixed: some show no change in INR with moderate grapefruit consumption, others show occasional INR elevations. The safer practical stance is: warfarin is already a narrow-therapeutic-index drug, and any new food or supplement deserves caution.
Practical guidance
- Moderate, consistent grapefruit intake: usually does not significantly change INR in stable patients — but requires an INR check when first adding or stopping.
- Binge grapefruit consumption (multiple glasses per day, suddenly): rare case reports of INR elevation and bleeding events exist — avoid.
- Grapefruit juice + warfarin: more potent interaction than fresh fruit, because juice concentrates furanocoumarins.
- Golden rule: keep your diet consistent. The same glass of juice every morning is easier on warfarin than irregular consumption.
- INR check timing: get an INR 3–7 days after any new regular food or supplement — including grapefruit.
