Side effects of pseudoephedrine includeinsomnia, elevatedheart rate, increasedblood pressure,restlessness,dizziness,anxiety, anddry mouth, among others.Rarely, pseudoephedrine has been associated with seriouscardiovascular complicationslikeheart attackandhemorrhagic stroke.Some people may be more sensitive to itscardiovasculareffects.Pseudoephedrine acts as anorepinephrine releasing agent, thereby indirectly activatingadrenergic receptors.As such, it is an indirectly actingsympathomimetic.Pseudoephedrine significantly crosses into thebrain, but has someperipheral selectivitydue to itshydrophilicity.Chemically, pseudoephedrine is asubstituted amphetamineand is closely related toephedrine,phenylpropanolamine, andamphetamine.It is the (1S,2S)-enantiomerofβ-hydroxy-N-methylamphetamine. Along with ephedrine, pseudoephedrine occursnaturallyinephedra, which has been used for thousands of years intraditional Chinese medicine.It was firstisolatedfrom ephedra in 1889.Subsequent to itssynthesisin the 1920s, pseudoephedrine was introduced for medical use as a decongestant.Pseudoephedrine is widely availableover-the-counter(OTC) in both single-drug andcombinationpreparations.Availability of pseudoephedrine has been restricted starting in 2005 as it can be used to synthesizemethamphetamine.Phenylephrinehas replaced pseudoephedrine in many over-the-counteroraldecongestant products.However, oral phenylephrine appears to be ineffective as a decongestant.In 2023, pseudoephedrine was the 292nd most commonly prescribed medication in the United States, with more than 400,000 prescriptions.In 2023, the combination withbrompheniramineanddextromethorphanwas the 281st most commonly prescribed medication in the United States, with more than 700,000 prescriptions.In 2023, the combination withloratadinewas the 300th most commonly prescribed medication in the United States, with more than 400,000 prescriptions.
